Disabilities Service Pathway Key Information

The Disability Service pathway describes considerations for practitioners when working with disabled children, young people and their families.  The considerations include communication tips, stress factors for families, parental disability, transition planning and recording requirements. Possible supports for families of disabled children or young people are also outlined, as well as information about out-of-home placements for disabled children under sections 141 and 142 of the Children, Young Persons and Their Families Act 1989.
