Police Checks

There is now a new Police Check form on the Practice Centre which is to be used when seeking consent from caregiver applicants or others such as employees. This new form will also be on CYRAS soon. When the form is signed it is sent to the Licensing and Vetting Service Centre in Wellington. Please note that Child, Youth and Family and Police now have agreement that the Vetting Centre will provide family violence history to CYF automatically. This means there is no requirement to ask separately for information regarding family violence history. Please do not use any old Police check forms, as this is the only one accepted by the Licensing and Vetting Centre.
